Management

Caregivers have many tasks when managing post-transplant care for their child. They manage medications, nutrition, incision care, etc. Caregivers are taught a variety of health care tasks that they may have never encountered before. Care centers provide a personalized guide on caring for the patient at home. Review his care binder with the caregiver to ensure they understand their treatment plan. If they have questions that cannot be answered by the information inside their care binder, loop in the liver team to explain to the caregiver. There are many ways to help a caregiver stay organized. Guide them in finding a method that best fits their learning style (electronic calendars, written planners, applications.)


Appointments

Helping the caregiver determine who to call for specific questions will help streamline their child's care and ensure their questions are being answered in a timely manner. Print out the following documents and fill them our with the caregiver.


Planners

A planner will help the caregiver stay organized with adminstering medications and appointment management. Assist the caregiver in finding a system that works best for them.

Childcare

All families are unique. Some caregivers may have other children that they need to arrange childcare for while they are in the hospital, attend clinic, or go to work. There are programs that caregivers could potentially use to help them care for their other children or the patient themseleves.

Discharge

Once the patient is recovering well, the transplant team will discharge them home. To help the caregiver feel confident goin home from the hospital, provide them with guides that include the numbers that they can call at your health care center if they have any questions while they are home. Ensure their MyChart is set up so they are able to communcate with their care team.

Insurance

There can be unexected situations that come up when managing insurance. An unexpected bill should always be reviewed and discussed with the insurance company or hospital to ensure the caregiver does not end up paying an unecessary bill. Transportation could potentially be reimbursed to the caregiver depending on their policy.
