School Communication

For patients that are of school age, it is important that the school is aware of the child's condition in order to not have the child fall behind academically. The health advocate should have a conversation with the caregiver to determine what the school is already informed of. There may be letters that the care team may provide the school to better inform them of the current health status of the patient and notify them of any modifcation that are needed for the benefit of the patient. There may also be other resouces the caregiver can use to ensure their child is receiving the support they require such as a 504 or an IEP plan.
