Finding resources
Families are faced with many unique challenges when caring for a post-liver transplant child. When you start a participant's intervention, ensure you are adressing immediate needs first. Immediate needs include food insecurity, housing insecurity, safety concerns, etc. There may be hospital resources that may be available to the patient. Always check-in with your clinic's social worker to determine if there are any hospital resources that could address their immediate needs.
You can also look for goverment programs that could adress the patient's social need (Food stamps, unemployment, IHSS, SSI, child care subsidized programs, etc.) Google searching will allow you to quickly access this information. You can also search for resources on google that are local to your patient by typing " Food resources near (enter zip code here.)" There are many resource lists that you will come across while searching for resources. Always insure to call the resource to verify they are currently still offering their assistance before sending the information over to the participant. There are also some condition specific grants that your patient may be eligible for. Ensure to disclose all information about the organization to the caregiver, and confirm wether or not they would like you to start an application before sending in an application on their behalf. Some organzations require information of the patient to be uploaded to their website in order to recieve their assistance (i.e. child or family picture,summary of transplant journey, reason for needing assistance.) Present all opportunities available to a patient, and let them decide wether or not it is truly a good fit for them.
